Patient Confidentiality

Your medical record is a confidential document.  It is the policy of this practice to maintain security of personal health information at all times. We ensure that this information is only available to authorised members of staff who, as a requirement of their employment, have signed a confidentiality agreement.

It is also a policy at this surgery that all patients over the age of 16 are required to contact the surgery for their own results. An authority form can be signed at the surgery to allow other family members to ring for results or to pick up any requested information.

How to obtain a copy of your records

Our practice acknowledges patients may request access to their medical records. We require you to put this request in writing and our practice will respond within a reasonable time. Our practice will undertake to forward the information requested within 10 working days of the fees being paid. Our fees are $35 per file copy and an account will be forwarded to you on receiving a written request.

For information requested by a third party, a signed authority by the patient will be required and the information will be forwarded on receipt of payment of account. The fees for this will be calculated at the AMA rate.
