We ensure that this information is only available to authorised\nmembers of staff who, as a requirement of their employment, have signed a\nconfidentiality agreement.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is also a policy at\nthis surgery that all patients over the age of 16 are required to contact the\nsurgery for their <strong>own<\/strong> results. An\nauthority form can be signed at the surgery to allow other family members to\nring for results or to pick up any requested information.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We abide by the ten National Privacy principles available at <strong><a href=\"http:\/\/www.privacy.gov.au\/health\/index.html\">http:\/\/www.privacy.gov.au\/health\/index.html<\/a><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">You can access our  Privacy Policy <a href=\"https:\/\/www.drpauleyssurgery.com.au\/forms\/\">here<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>How to obtain a copy of your records<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Our practice acknowledges patients may request access to their medical records. We require you to put this request in writing and our practice will respond within a reasonable time. Our practice will undertake to forward the information requested within 10 working days of the fees being paid. Our fees are $35 per file copy and an account will be forwarded to you on receiving a written request. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For information requested by a third party, a signed authority by the patient will be required and the information will be forwarded on receipt of payment of account.
